Lumenos, a division of WellPoint, offers HSA- and HRA-based plans. Anthem ByDesign Portfolio offers integrated HRA- and HSA-based plans. PowerHealth Fund is an integrated HRA and HSA available in Calif., Mo., Wis. and Ga.

National offerings available through Lumenos. Anthem ByDesign and PowerHealth Fund available for small to national accounts. HSA product available for employers of all sizes and individuals.

Combined enrollment in HRA- and HSA-based plans is 1.2 million as of June 30.

Enrollment: Combined with HRA enrollment.

Wellmark Blue Cross and Blue Shield of Iowa and Wellmark Blue Cross and Blue Shield of South Dakota

Blue Priority HSA combines HDHP in PPO network with an HSA. HRAs are available for larger employers and can be combined with any type of plan, typically one with a deductible of $1,000 or more.

All markets, large groups, small groups and the individual market.
